Checklist for Completing the Evaluation Form (MD's Only)

  1. The 4 questions at the top of the Evaluation Form MUST be completed.

  2. All 20 questions evaluating your meeting weekend MUST be completed.

  3. Submit your evaluation form to our Administrator:

Please note that any Evaluation Form returned without a legibly written name will be DISCARDED.

  • Please expect 2-4 weeks for your certificate if sent via e-mail or fax.

  • Please expect 6-8 weeks for your certificate if sent via snail mail.
